To help seniors living in our community be more independent and active by bringing proactive PT and OT services into the home.

Our History

In 2018, husband and wife team Holly and Steve created Extend to fill a void in Therapy Services available to Seniors living in the Community.

Our Patients: Many of our patients are not mobile enough to regularly make it to an Outpatient Clinic and prefer the convenience and one-on-one attention of Therapy at home. For some Extend patients who live alone with limited access to outside help, our Therapists are primarily responsible for keeping them healthy, safe and comfortable at home.

By carefully listening to and following through on the needs of our patients, Extend has become the trusted way to make possible what was once believed to be impossible. We often hear from our patients about how our Extend therapists "have become just like family."

Our Therapists: Extend Physical and Occupational Therapists have true autonomy to provide clinically excellent care using evidence based interventions and best practice guidelines in the home environment. Our Therapists have many combined years experience in Outpatient and Homecare settings clinicians and are all certified in Otago Fall Prevention.

Our Partners: We work together with Families, Physicians, Independent Livings, Assisted Livings, Primary Caregivers and Home Aides to make sure our patients are getting access to the quality Therapy Services that they need.
